Student Complaint and Reporting Form:
Prairie State College fosters an open educational environment, rich in values and is designed to protect and uphold the integrity of teaching and learning. Students are encouraged to first address complaints and concerns directly to the faculty, staff, or relevant administrator.
Student of Concern
Please use this form if you have concerns about the well-being of a Prairie State College student or are concerned that a student's behavior may violate a College policy.
Threat Assessment Report
The Threat Assessment Team reviews significant behavioral incidents and student misconduct that may violate PSC policies and the Code of Conduct. Reports are reviewed daily during normal college business hours. Anyone can submit a report, which can be accessed through the link below.
Tuition Appeal Form
Use this form to request a waiver of Prairie State College policies due to extenuating circumstances. Provide necessary documentation for full or partial tuition credit. Decision notification via email may take up to four (4) weeks.
Petition for Academic Reinstatement
To request reinstatement to Prairie State College, you must complete this form and submit a detailed summary, which the Dean's Committee on Reinstatement will review. Please anticipate a processing period of at least four (4) weeks for your appeal to be considered.

Title IX Report Form - Students
Prairie State College is dedicated to maintaining a harassment-free educational environment. Non-confidential employees are Campus Security Authorities (CSAs) mandated to report gender-based discrimination and harassment. Use this form to report incidents such as stalking, violence, assault, harassment, and discrimination. For accurate reporting, provide factual details rather than opinions. Please ensure you fully complete and submit the appeal form below within five (5) business days of receiving the hearing decision. Failure to meet this deadline will result in the appeal being denied.
Pregnancy-Related Student Accomodations
Prairie State College ensures equal access for pregnant and parenting students under Title IX, prohibiting discrimination based on sex in all educational programs and activities receiving federal funding. Pregnant students can request accommodations by contacting the Title IX Coordinator promptly, providing medical information directly to them for evaluation.
